PATH for Women Scholarship Fund for Pacific Islander and Southeast Asian Students

Overview

REACH US – Promoting Access to Health for Pacific Islander and Southeast Asian Women (PATH for Women), a breast and cervical cancer program working to address disparities among Asian and Pacific Islander communities, is offering the PATH for Women Scholarship Fund to Cambodian, Chamorro, Hmong, Laotian, Marshallese, Native Hawaiian, Samoan, Thai, Tongan, and Vietnamese high school seniors and undergraduate college students who demonstrate significant merit in academics, workplace, and community with an interest in the health field. Twelve students will be selected to receive a $1,000 scholarship to offset their expenses at a two‐or four‐year accredited institution. The scholarship is funded by the PATH for Women partnership which includes the following organizations:

Families in Good Health/St. Mary Medical Center, Guam Communications Network, Orange County Asian Pacific Islander Community Alliance, Pacific Islander Health Partnership, Samoan National Nurses Association, Tongan Community Service Center/Special Service for Groups, and Union of Pan Asian Communities. This scholarship opportunity is supported by Cooperative Agreement Number 5U58DP001006 from the Center for Disease Control and Prevention (CDC) through the Racial and Ethnic Approaches to Community Health (REACH) across the United States (US) funding opportunity. REACH US PATH for Women is a program of the Orange County Asian and Pacific Islander Community Alliance (OCAPICA). Low income students and those who are of the first generation in their family to attend college are encouraged to apply.



Eligibility

• High school seniors and students who are currently or will be attending a two or four year
institution that offers accredited, post‐secondary instruction in Fall 2011 may apply.
• Applicants must be Cambodian, Chamorro, Hmong, Laotian, Marshallese, Native Hawaiian,
Samoan, Thai, Tongan, or Vietnamese.
• Must reside or attend college in Orange County, Los Angeles County or San Diego County.
• Must demonstrate an interest in a health field.
• Students who have received this scholarship in prior years are NOT eligible for this year’s
scholarship.

Application Requirements

‐Interested students must submit a complete application and the required documentation.
‐Applications must be TURNED IN no later than 5pm on Monday, May 16, 2011.

Scholarship Terms & Conditions

The PATH for Women Scholarship Fund will award twelve $1,000 scholarships, which will be distributed to help offset the educational costs for the 2011‐2012 school year. Scholarship winners will be notified by the first week of July 2011.

Scholarship recipients must be enrolled by Fall 2011 at an accredited institution in the academic year of their selection. Scholarship funds not claimed in the academic year will revert back to PATH for Women.

Scholarship funds will go directly to the school, not to the student. The funds will be disbursed toward tuition, fees, and other appropriate educational expenses. Funds will be disbursed to schools in August 2011.


Certification & Release

All applicants, and where appropriate their parent or guardian, must sign the application certifying that all information provided is true and complete to the best of their knowledge and ability. With their signature and submission of the application, applicants also grant PATH for Women, OCAPICA, and other partners the right to use their information and their likeness for the purpose of promoting and publicizing the PATH for Women Scholarship Fund. Applicants under the age of 18 must have their parent or guardian sign the application.

All complete applications must be postmarked by 5pm on Monday, May 16, 2011. Incomplete
application packets will be disqualified. Applications must be sent to:
