USA: Carolina Undergraduate Health Fellowship 2010

Overview

The Carolina Undergraduate Health Fellowship (CUHF) enables promising UNC undergraduates to create a self-designed health-related project anywhere in the world. The fellowship is an investment in talented students whose medical and cultural experience will strengthen the future of health care both in the United States and the world.

The goals of CUHF are to prepare students to become successful leaders in the health care profession by:

Exposing them to practical (rather than purely observational) health care scenarios
Increasing their cultural sensitivity and adaptability through direct interaction with unfamiliar cultures and people
Presenting them with experience with different health care and medical systems

Eligibility

Any returning UNC undergraduate may apply
Preference is given to applicants whose financial needs would otherwise not allow them to pursue such a project
Successful applicants are required to give at least one presentation on the project findings at the Center for Global Initiatives’ Annual Research Symposium and must be available to serve on the CUHF Selection Committee the following spring term

Restrictions

Students who apply for the CUHF may NOT also apply to the following Center for Global Initiatives awards:

•Internship Award
•CV Starr Scholarship

Because there is some overlap between these funding opportunities, students must choose only one for which to apply during any given academic year.


Available Funding

Estimated Available Funds: $12,000. Typical Award Amount: $1,000-$3,000.

Typical Number of Awards: 5 Typical

Number of Applicants: 16

The Center for Global Initiatives is not bound by these estimates and is often unable to award the full amount of funds requested. Each award is made as a stipend, issued as a check.

Application ProcessThe application is available through our Online Application Center. Please note that the online application will only accept attachments in PDF format. The newest versions of Microsoft Word allow you to save your documents in PDF format, or you may visit http://www.freepdfconvert.com/ or http://www.bullzip.com// for free conversion.

Materials which must be submitted by the deadline, via the ONLINE APPLICATION CENTER are:

Part 1:

Basic demographic application data

Part 2:

Project title and abstract
A project narrative in PDF format
- The narrative should examine your proposed project through the lens of the evaluation rubric, below.
- Maximum 1000 words.
Resume/CV in PDF format
Project start and end dates
Total amount requested from CGI

Budget that details actual travel and subsistence costsYour line item budget may be more or less than the amount you request from CGI.
List of sources of alternative funding requested and received

Part 3:

Please enter the contact information for the letters of recommendation and the letter of affiliation(if needed); see hard-copy materials below.

In addition to the online application, please drop off the following HARD-COPY materials to the Center for Global Initiatives by the deadline in a large clasp envelope. You should have your letter writers give/send the letters to you sealed and signed across the back for you to hand-deliver to the center with your transcript. Letters and transcripts must be SINGLE SIDED and UNSTAPLED. Please type or write your Application ID number from the ONLINE APPLICATION CENTER on each item and on the clasp envelope.

( A special box for the CV Starr applications will be located in our reception area (suite 3002) the week of the deadline.)


•Letter of recommendation from a UNC faculty member.
It is in your interest that this letter is specific to your proposed project.
•Letter of recommendation from a health-care practitioner.
It is in your interest that this letter is specific to your proposed project.
•A letter of affiliation from a contact at the organization where you will conduct your project.
This is only required for projects that are experiential (work, internship, etc.) in nature.
•Current UNC transcript.
An unofficial transcript from Student Central is acceptable, or you may request a free internal transcript from the registrars office.

Notification of Award

Applicants will receive award or rejection notices within 4 weeks of the deadline via email or USPS mail. Do not contact the Center for Global Initiatives to inquire about the status of your application during this time frame. However, If you have not received a notice 8 weeks after the application deadline, please inform us.

Award RequirementsAll awardees are responsible for securing proper IRB approval for their projects, if required. The Office of Sponsored Research and the Office of Undergraduate Research can assist you in determining if IRB approval is required.

CGI embraces a culture of "giving-back." If selected for funding, awardees must choose both a service and a research/academic requirement from among a menu of options. You can read the general "giving-back" guidelines here (available soon).

Please note that these are subject to change and will be communicated to you at a special information session for award winners.

In addition, if you will be traveling to Asia, Africa, or Latin America, you MUST attend the GO! Global Orientation of Culture and Ethics Pre-Departure event on April 17, 2010. There are no exceptions permitted. Please reserve this date in your calendar at the time of application.

Finally, you must also complete a very brief online report though our application center at the completion of the project.
